Longevity Testing: Adding Healthy Years to Your Life

Singapore residents have one of the longest life expectancies at birth in the world at 83.9 years as of 2025, and was recently recognized as a Blue Zone, one of the few regions on earth with an unusually high concentration of people who live past 100. But there's a quieter number behind that headline: health adjusted life expectancy (HALE) here is 73.6 years based on data from 1990 to 2021, where HALE refers to the average number of years that a person can expect to live in “full health” from birth. 

The ten-year gap between these two statistics is what researchers call the difference between lifespan and healthspan, the years you're alive, versus the years you're alive and in good health.

Generally, standard checkups are built to catch problems once they've already started. But health longevity testing works differently: it looks for the early signals that precede disease by years, while there's still time to change the trajectory.

Why "normal" results don't always mean low risk

A standard cholesterol panel, a fasting glucose test, and a basic health checkup helps to pick up medical conditions. But they're also designed with fairly wide normal ranges, which means early-stage risk can sit comfortably inside "normal" for years before it becomes a diagnosis. Health longevity testing narrows the lens, using more specific markers to catch what standard panels are built to miss.

The systems worth understanding

1. Metabolic Health: How you power your day 

This is about how your body converts food into energy, process the nutrients, and keeps your blood sugar, cholesterol and other metabolic markers within an optimal range. For example, testing with fasting glucose and HbA1c (your three-month blood sugar average) will eventually detect diabetes and prediabetes, but they often miss the years of insulin resistance that precede diabetes because the body compensates by producing more insulin to keep glucose readings looking normal. Testing fasting insulin alongside glucose, and calculating HOMA-IR, can flag that insulin resistance while it's still easy to act on. A body composition scan rounds this out by showing how that energy balance is actually playing out physically.

2. Cardiovascular Health: Built for the long run 

Heart disease is Singapore's leading cause of death, and it's a major driver of lost healthspan, not just lost lifespan. The standard panel (total cholesterol, HDL, LDL, and triglycerides) is the starting point, but it's often not the full picture. Apolipoproteins B and A1 measure the actual particles carrying bad and good cholesterol through the bloodstream and are frequently more precise predictors of risk than LDL and HDL cholesterol alone. Lipoprotein(a) reveals inherited cardiovascular risk that lifestyle changes can't fully address, hs-CRP flags the inflammation that contributes to arterial damage, and NT-proBNP can detect early strain on the heart itself. Blood pressure, heart rate,, and VO2max (a measure of aerobic fitness) complete the picture of how well your heart and vessels are actually coping with daily demands.

3. Inflammation and Recovery: Bouncing back 

How well your body resolves inflammation after stress, training, or illness shapes how quickly you recover and how strong your immunity stays. hs-CRP flags background inflammation, white blood cell indices show how your immune system is currently allocated, and Vitamin B12, Folic acid, Vitamin D, , zinc, and magnesium levels reveal whether your body has what it needs to actually recover rather than just cope.

4. Hormones and Vitality: The messengers behind how you feel 

Testosterone in men, FSH in women, and thyroid hormones (TSH, Free T3, Free T4) influence energy, mood, libido, and metabolism in ways that are easy to misattribute to "just getting older." When these markers drift, the effects tend to show up everywhere else first (energy, sleep, motivation, muscle mass) before anyone thinks to check the hormones themselves.

5. Cognitive and Mood Support: Mental clarity 

Vitamin B12 and magnesium both play a direct role in brain and nerve function, and deficiencies in either can present vaguely as mood issues, and poorer mental function. A Mild Cognitive Impairment (mental function) screening test can add a functional check on top of the bloodwork, assessing cognitive performance directly rather than inferring it from nutrient levels alone.

6. Nutrient Balance and Capacity: Running on full 

Deficiencies rarely announce themselves clearly, and food intake tends to decline with age, making them easier to miss over time. Iron studies (TIBC, transferrin, iron saturation) and ferritin assess your iron stores; zinc, magnesium, vitamin B12, and folic acid cover core cellular and nerve function; vitamin D, calcium, and phosphate support bone and metabolic health; and albumin and globulin reflect your body's broader protein balance. Low levels in any of these can quietly affect energy, brain and nerve function, skin, hair, and immunity long before they become an obvious problem.

7. Liver Health: Keeping the system clean 

ALT, AST, ALP, GGT, and bilirubin are the core markers of how well your liver is functioning and metabolizing nutrients and medications, and clearing waste. Albumin, globulin, and the Albumin/Globulin ratio add context on liver protein production. Hepatitis A and B screening rounds this out, since these  infections can stress the liver, and also identifies opportunities to go for vaccination for early protection of the liver.

8. Kidney Health: Staying in balance 

Creatinine and eGFR measure how well your kidneys functioning to filter waste, while sodium, potassium, calcium, chloride, and bicarbonate show whether your electrolytes are in balance. A urine dipstick test and microscopy (checking pH, protein, glucose, and cells) can flag early kidney and urological tract changes that wouldn't otherwise be felt at all — kidney function tends to decline silently, well before symptoms show up.

9. Cancer Testing: One more layer of awareness 

Certain blood markers can rise in connection with specific cancers: CEA, AFP, CA 19-9, CA 125 (for women), PSA (for men), and EBV IgA, alongside broader options like the Lucence multi-cancer blood test. It's worth being clear about what these can and can't do — on their own, none of them can confirm or rule out cancer. They're most useful as one additional signal alongside recommended, age-appropriate screening, not a replacement for it.

10. Longevity and Biological Age: How fast you're actually ageing 

This is where the other nine systems come together. LinAge 2.0 estimates biological age (how old your body is) based on a broad set of underlying markers. Biological age helps to predict all-cause mortality, better reflecting the overall impact of various organ systems and disease  Functional capacity test measures, such as handgrip strength, the 5x sit-to-stand test, assess physical function directly rather than inferring it from blood work. VO2max and body composition round this out, tying the biological picture to how your body is actually performing day to day, and how your muscles and body fat are distributed.

How testing actually works

Health longevity blood tests require a doctor's order, so the process starts with a consultation rather than a lab visit. A doctor reviews your medical history, symptoms, and risk factors, then determines which markers are actually relevant to you. Not everyone needs every test on this list, and some people need tests beyond it. Many of these tests require fasting, though the blood draw itself can typically be scheduled separately from your initial consultation. Results are usually available within a few days.

What the results are actually for

No single test, or combination of tests, can tell you exactly how long you'll live. Health longevity markers are correlations, not predictions. What they can do is flag which conditions you're more likely to develop, early enough that lifestyle changes, supplementation, or medication can meaningfully shift your risk before symptoms appear.

That's also why testing works best as a repeated process rather than a one-off. Markers shift over time, and re-testing periodically lets you and your doctor see whether your current intervention approach is actually working and adjust the plan if it isn't.

Adding healthy years to life isn't about chasing immortality or the latest longevity trend. It's about protecting the things that matter most – your energy, mobility, independence and ability to enjoy life with the people around you. The goal isn't simply a longer life. It's a healthier one.
