Adj Associate Professor Sue-Anne Toh
Senior Consultant, Endocrinology
Founder, Chief Executive Officer & Medical Director
Dr Sue-Anne Toh is a healthcare leader with over 20 years' experience in clinical care, academic research and health systems administration. She is an internationally recognized key opinion leader in diabetes, metabolic health, weight management and population health.
She is Adjunct Associate Professor, Yong Loo Lin School of Medicine, National University of Singapore, Honorary Senior Consultant and Director, National University Hospital and founding Director of the Singapore Population Health Improvement Centre (SPHERiC).
She was formerly Senior Consultant, Endocrinology, at the National University Hospital (NUH) and Director, Regional Health System Office, National University Health System (NUHS).
Dr Toh graduated from the University of Cambridge with honours in 2002 and received her postgraduate qualifications from the American Board of Internal Medicine. She also holds a Master’s in Translational Research from the University of Pennsylvania.

Adj Associate Professor Kyle Tan
Consultant, Preventive Medicine

Dr Kyle Tan is a Preventive Medicine Specialist with over 10 years of clinical experience spanning primary care, tertiary hospitals, and community health.
He has designed and implemented large-scale preventive health and chronic disease management programmes, and served in population health planning roles at NUHS.
He continues to contribute to academic teaching as Adjunct Assistant Professor at NUS Saw Swee Hock School of Public Health.

Dr Todd On
Associate Consultant, Family Physician
Dr Todd On is an Associate Consultant, Family Physician with 13 years of clinical experience in tertiary hospital and community settings.
He has cared for patients across age groups, collaborating with multidisciplinary teams to deliver person-centred care for both acute and chronic conditions.
Dr On has led multiple quality improvement projects, including a smoking cessation programme and a collaboration to reduce lower limb amputation in diabetic patients.
He is also passionate about teaching, serving as a Faculty Tutor and Mentor at the College of Family Physicians Singapore, where he has received multiple teaching awards.

Dr Yeo Junjie
Family Physician
Dr Yeo Junjie is a Family Physician with clinical expertise in chronic disease and weight management.
He holds a Doctor of Medicine degree from Duke-NUS and a Graduate Diploma in Family Medicine.
Dr Yeo has experience in managing such conditions as obesity, diabetes, hypertension and hyperlipidemia.
He has been involved in preventive and public education initiatives, including community health screenings and talks.

She holds a Bachelor's degree in Nutritional Sciences from Rutgers University and a Master of Public Health from the University of Leeds

Mee Li Yap

Mee Li is a Senior Dietitian with over 10 years of clinical experience, focusing on diabetes management and lifestyle interventions.
She has served as the Clinical Lead Dietitian for Diabetes at KTPH Diabetes Centre and has been involved in research and quality improvement projects.
Mee Li is passionate about using medical nutrition therapy to help individuals with various medical conditions.

Elizabeth Sin Tzun Chong

Elizabeth is a Senior Dietitian with over 10 years of experience in medical nutrition therapy, particularly in managing chronic conditions like diabetes and hypertension.
She specializes in renal nutrition and is passionate about nutrition education and advocacy.

Joel Chia

Joel is a Senior Dietitian with over 7 years of experience in acute settings, including sports nutrition, weight management, and chronic disease management.
He has been involved in global initiatives promoting physical activity and has served as a Clinical Supervisor for student dietitians.
Joel holds a First-Class Honours Bachelor's Degree in Nutrition & Dietetics from Flinders University, a Postgraduate Diploma in Sports Nutrition from the Institute of Performance Nutrition, and is certified in several exercise-related disciplines.

Charlotte Tan
Senior Dietitian
Charlotte is a senior dietitian with over 7 years of clinical experience in the acute hospital setting managing chronic disease, seeing both inpatients and outpatients.
She specializes in weight management as well as dietary management of bariatric patients and is passionate about sharing her nutrition knowledge with the public through talks and community events.
She has also been an invited speaker at various medical conferences.
She holds a Bachelor's degree in Food Science and Technology from the National University of Singapore and a Master degree in Dietetics from Curtin University.

Sharon Gill
Dietitian
Sharon is a Dietitian with over 5 years of experience in clinical dietetics, food services, and community dietetics.
She is a Registered Dietitian with the Ministry of Health (MOH) Malaysia and a member of the Malaysian Dietitians’ Association (MDA).
Sharon has worked at private hospitals in Kuala Lumpur, managing a diverse range of cases, such as obesity, type 2 diabetes, and hyperlipidemia, as well as geriatric, cancer, critical care, and pediatric patients.
She has led health and wellness workshops, delivered health talks, and conducted live healthy cooking demonstrations.
Sharon believes in offering sustainable solutions for her patients.
She holds a Bachelor’s degree in Dietetics with Nutrition from IMU University, Malaysia.

Chad Han

Chad is a Senior Dietitian and an Advanced Accredited Practising Dietitian (AdvAPD) with Dietitians Australia.
He has over 12 years of experience in medical nutrition therapy across Singapore and Australia, with a PhD in nutrition and dietetics from Flinders University.
Chad has published extensively on topics like telehealth in diabetes management and is actively researching ways to improve health services for chronic diseases.
