Dr Sue-Anne Toh is a Senior Consultant Endocrinologist with almost 20 years’ experience in internal medicine, diabetes and metabolism. She obtained her medical degree from the University of Cambridge, UK. Her postgraduate training was at the University of Pennsylvania, Philadelphia, USA, where she attained her specialist qualifications in Internal Medicine and Endocrinology (American Board of Internal Medicine), as well as a Master of Science in Translational Research.

As part of her medical training, Dr Toh completed rotations in Addenbrooke’s Hospital, and Beth Israel Deaconess Medical Center and Massachusetts General Hospital, all renowned teaching hospitals of Cambridge University, UK, and Harvard University, USA respectively.

She also held an appointment as Postdoctoral Clinical Research Scholar, Instructor of Medicine (Physician Scientist Track) at the Institute for Translational Medicine and Therapeutics, University of Pennsylvania, while serving as Attending Physician (Consultant) at the Hospital of the University of Pennsylvania, USA.

After returning to Singapore, Dr Toh served as Senior Consultant, Endocrinology, at the National University Hospital (NUH), where she led person-centred care models to improve care integration and outcomes, particularly for patients with complex medical conditions.

Dr Toh specializes in the management of a wide spectrum of conditions including diabetes, lipid disorders, metabolism, weight management and cardiovascular risk management.

Dr Toh has been invited to speak at numerous local and international conferences and Continuing Medical Education programmes. She is also a member of many national clinical, research and innovation workgroups, such as the Ministry of Health Lipid Clinical Practice Guidelines Workgroup, the national Diabetes Research Taskforce, the Ministry of Health National Diabetes Database Workgroup, the Ministry of Health Regional Health System Implementation Workgroup, the Ministry of Health Holdings (MOHH) Telehealth Implementation Workgroup, and the Precision Medicine: Clinical Practice Adoption Workgroup. In addition, she serves on the advisory boards of pharmaceutical, biotech and life science industry firms.

Dr Toh is a recipient of national and international awards including American Heart Association Awards, Singapore’s National Medical Research Council Clinician Scientist Award, Public Service (PS21) Gold Award, National Healthcare Innovation and Productivity Excellence Champion Medal, the University of Pennsylvania’s Institute of Translational Medicine and Therapeutics Fellowship Award and the Cambridge Commonwealth Society Fellowship.

Dr Toh’s translational research focuses on functional characterization of pathways involved in type 2 diabetes and cardiovascular complications. She continues to be active in research as an Honorary Visiting Consultant at the National University Hospital (NUH) and Yong Loo Lin School of Medicine, National University of Singapore (NUS), and she is the Principal Investigator for the Biobank and Registry for StratIfication and Targeted IntErventions in the Spectrum Of Type 2 Diabetes (BRITE-SPOT) and Assessing Progression to Type 2 Diabetes (APT-2D) cohorts, which study persons with, or at risk for, type 2 diabetes. APT-2D is a large comprehensive scientific study on progression of diabetes in Asian populations. 

She has successfully secured multi-million dollar competitive research funding from the National Medical Research Council of Singapore, as well as from industry. She is also well published in journals such as Immunity, Cell Metabolism, Journal of Clinical Investigation, Nature and Science.

Dr Toh was formerly Director, Regional Health System Planning Office, National University Health System (NUHS), driving population health initiatives to achieve a healthy and engaged community. She remains the founding Director of the Singapore Population Health Improvement Centre (SPHERiC), which was established in 2017 to be a centre for population health research.

Dr Toh has been an enthusiastic and passionate teacher and mentor for medical students, PhD students, post-doctoral fellows, medical interns, residents and Endocrinology senior residents for more than ten years, and believes it is a great joy, responsibility and privilege to be in a position to inspire, influence and help them to achieve their full potential. Several of her mentees have progressed to leadership positions in their fields, and have been successful in receiving awards at national and international conferences for the impact of their work. In addition, she is actively engaged in committees at the National University of Singapore which shape undergraduate and postgraduate medical education, and continues to serve on Thesis Advisory Committees for several PhD students. While at NUHS, Dr Toh also actively guided and mentored senior nurses, and supported the development of the advanced nurse practitioner-led CardioMetabolic Clinic (ACDC) Program in close collaboration with the National University Heart Centre, Singapore (NUHCS).

In her clinical practice, she places a great emphasis on listening to her patients and fully understanding their  lifestyles and priorities, so that she can tailor a holistic and evidence-based care plan that helps them better control their conditions and enjoy a good quality of life. Dr Toh has been a member of the Endocrine and Metabolic Society of Singapore (EMSS), the American Association of Clinical Endocrinologists, the American Diabetes Association and the American Heart Association.

Dr Kyle Tan is a Preventive Medicine Specialist. He obtained his medical degree from the National University of Singapore (NUS) in 2010, his Master of Public Health from NUS in 2014, and completed his postgraduate specialisation in Preventive Medicine with the National University Health System (NUHS) in 2016. He was appointed the Chief Resident of the Preventive Medicine Residency programme in 2015. He has more than 10 years of clinical experience and has worked in the medical, surgical, and paediatric departments in the National University Hospital as well as in the polyclinics. Dr Tan has a keen interest in chronic disease prevention and management and has been involved in the design and execution of multiple screening, preventive health and chronic disease management programmes at the regional health system and national levels.

As a lifelong technology enthusiast, he has a strong interest in using technology to incorporate personalized health coaching, nutrition plans and fitness advice into standard care for lifestyle-related medical conditions, to help individuals better understand and improve their health. Dr Tan also has significant experience developing and implementing healthcare policies at organisational and national levels. He was formerly Deputy Head (Planning and Development), Regional Health System Planning Office and Consultant in Preventive Medicine at the NUHS, where he shaped NUHS’ population health strategy and drove the development and system-wide implementation of preventive care. As an Assistant Director in the Ageing Planning Office of the Ministry of Health, Singapore, Dr Tan was a member of the core team that planned policies and developed services in wellness, active ageing and aged care, including developing a S$3 billion Action Plan for Successful Ageing in Singapore. This work involved designing and coordinating national policies across 12 government agencies as well as companies and NGOs to address ageing-related issues, including learning, employment, health, aged care, retirement adequacy, social isolation, transport and infrastructure. For this work, Dr Tan was a recipient of the Permanent Secretary for Health Award for his national-level contribution.

Dr Tan also served as a Staff Officer in the Biodefence Centre, Headquarters Medical Corps, Singapore Armed Forces (SAF), where he was the primary officer responsible for planning and development of biodefence capabilities within the SAF’s Medical Corps. He also worked closely with the Ministry of Defence and defence scientists to shape the biodefence strategy of the SAF. In addition, he revised the SAF pandemic response framework and plans for medical and military operations, which were approved and implemented by SAF senior management. As a Senior Resident in Clinical Services in Changi General Hospital, he was part of the team responsible for quality improvement and compliance to regulations and Joint Accreditation International (JCI) standards. 

Dr Tan has been invited to speak at numerous panels and conferences on topics ranging from preventive care, healthy ageing, population health, care integration and the use of technology in healthcare. He was an appointed member of several national level workgroups and taskforces, including the National Interagency Haze Taskforce and Regional Health System Implementation Workgroup. In 2015, he was also invited to co-develop a framework for pandemic influenza risk management in Indonesia by WHO Indonesia and the Ministry of Health, Republic of Indonesia.

Dr Tan continues to be Adjunct Assistant Professor at the Saw Swee Hock School of Public Health at NUS, where he leads a data analytics research team. His research focuses on the use of big data to better risk-stratify individuals and develop predictive models to identify high risk individuals for targeted interventions. He has published on numerous topics, including infectious disease management and prevention, use of big data in healthcare, as well as identification of high-risk patients in a health system. He has also served as a mentor and tutor for younger generations of preventive medicine residents, and he enjoys sharing with them how public health concepts are translated to concrete programmes and policies in both the public and private sector.

Dr Yeo Junjie is a Family Physician with a keen interest in chronic disease and weight management. He completed the Post Baccalaureate Premedical Program at Columbia University in the City of New York in 2012 and later graduated from the Duke-National University of Singapore Graduate Medical School with a Doctor of Medicine in 2017. Dr Yeo subsequently obtained his Graduate Diploma in Family Medicine in 2020 with the College of Family Physicians.

Dr Yeo is experienced in both inpatient and outpatient care. He spent his initial training at the anesthesia department of Singapore General Hospital and Sengkang General Hospital where he worked in preoperative, operative as well as intensive care settings. Subsequently, Dr Yeo joined the National Healthcare Group Polyclinics where he focused on chronic disease management. Prior to joining NOVI Health, he worked at a private clinic where he managed a wide range of acute and chronic medical conditions, as well as carried out health screenings.

Dr Yeo is experienced in the management of chronic conditions such as obesity, diabetes, hypertension and hyperlipidemia. Over the course of his career, he witnessed firsthand the long-term sequelae and complications of common chronic conditions that affect many patients in Singapore. This drove his passion to emphasize preventive and holistic care early in the course of the care he provides to his patients, so as to help them achieve optimal outcomes.  He is a strong advocate of personalized care that takes into account each patient and their unique context, and also believes in developing strong supportive relationships with his patients to help them reach and maintain their health goals.

Dr Yeo has also contributed to the community by carrying out public education and outreach programs, including a radio talk show on topics pertaining to health and prevention such as vaccination, as well as participation in community-based health screening events.

Dr Yeo is a recipient of the SingHealth Best House Officer Award, and he was also awarded the Duke NUS Medical Student Fellowship Award and the Duke NUS Achievement Prize for Most Outstanding Third Year Translational Science Research Thesis for his research elucidating the genetic basis of acute and chronic pain in our local population.

Dr Todd On is an Associate Consultant, Family Physician. He graduated from the Yong Loo Lin School of Medicine, National University of Singapore (NUS) in 2010 and obtained his Graduate Diploma in Family Medicine (GDFM) in 2015 and Master of Medicine in Family Medicine (MMed (FM)) in 2017.  He was awarded the Collegiate Membership MCFP(S) by election in 2018 by the College of Family Physicians (CFPS).

Dr On brings with him 13 years of clinical experience. He has spent more than 9 years working in the National Healthcare Group (NHG) Polyclinics, as well as worked in the medical, surgical and paediatric departments at the National University Hospital (NUH). While at NHG Polyclinics, he was the Team Leader of his Teamlet, where he led doctors, nurses and care coordinators, and partnered with pharmacists and allied health professionals across the health and social sectors to deliver person-centred care for people with chronic conditions. He has broad clinical experience in managing paediatric, adult and geriatric acute and chronic medical conditions, women’s health, and mental health. 

As a doctor with a keen interest in systems-based practice who is constantly looking for ways to deliver better care to patients, Dr On has participated in, and led, multiple quality improvement projects, including developing and leading the Health Promotion Board (HPB) IQuit Campaign to promote nationwide quitting of smoking, and working with Tan Tock Seng Hospital in a cross-institution collaboration to reduce lower limb amputation rates in people living with diabetes. Most recently, Dr On became a Super User to guide doctors in Polyclinics as the public healthcare clusters transitioned to the Next Generation Electronic Medical Records (NGEMR) System. He also led engagement efforts to bring Family Physicians on board HealthierSG efforts.

Dr On has always made patient safety his top priority. In his previous capacity as the deputy lead of the Infectious Disease Specialty Advisory Group (ID SAG), he contributed to the development of antibiotic usage and infectious diseases treatment guidelines for the Polyclinics. He was also a member of the Medication Management and Usage Committee (MMUC) for the Polyclinics, where he helped improve medication safety by developing guides for the prescription of new medications, initiatives to address polypharmacy in the elderly, and protocols to facilitate the national recall of medications. In recognition of his contribution to work practices that improved patient safety, he was awarded the National Healthcare Group (NHG) Quality Day Excellence in Action Award (Individual).

During the start of the COVID-19 pandemic in Singapore, Dr On was among the first batch of doctors to volunteer in the foreign worker dormitories, where he set up and led a Medical Post within a dormitory, working with a team of nurses, pharmacists and swabbers to address migrant workers’ acute, chronic and mental health needs. For his dedication and efforts during the nation’s fight against COVID-19, Dr On received The Commendation Medal (COVID-19) conferred by the Prime Minister’s Office (PMO) Singapore.

Passionate about teaching and developing the next generation of Family Physicians, Dr On is a tutor and mentor for CFPS for the GDFM and MMed (FM) programmes, and was the previous lead of the Consults Faculty Tutor for the MMed (FM), CFPS College Programme. Dr On’s passion for teaching not only spans postgraduate but also undergraduate medical education. He was an adjunct lecturer with the Lee Kong Chian School of Medicine (LKCMedicine) and a clinical teacher for the Yong Loo Lin School of Medicine (NUS). In recognition of his teaching efforts and grooming the next generation of doctors, he has received multiple teaching awards including the NUS Yong Loo Lin School of Medicine (NUS) Dean’s Award for Teaching Excellence, and continues to mentor many today.
