Semaglutide, Liraglutide and other GLP-1 RAs help promote weight loss and keep blood sugar under control. However, there are different GLP-1 RAs that are licensed for specific purposes.
In Singapore, there are versions of semaglutide (Wegovy) and liraglutide (Saxenda) licensed for weight loss. There are also versions of semaglutide (Ozempic & Rybelsus), liraglutide (Victoza), and dulaglutide (Trulicity) licensed specifically for treating diabetes.

GLP-1 RAs help you lose weight by:
Reducing cravings. GLP-1 RAs overhauls your “hunger and craving hormones”, working on the reward center of the brain.
Suppressing appetite. The medication slows down gastric emptying, this makes you eat less, feel full longer, and lose weight.
Preventing overeating. By mimicking the GLP-1 hormone, it lowers blood sugar, reducing cravings and slowing digestion, preventing you from overeating.

In multiple clinical studies of liraglutide, using the medication consistently resulted in weight loss of 4 to 6 kg. Nearly two-thirds of patients lost at least 5% of their body weight, and nearly one-third lost at least 10%. Meanwhile, with semaglutide, patients achieved 5%–20% or greater weight loss.

When people first start to take GLP-1 RAs, they tend to lose weight rather quickly. After a few months, the weight loss starts to slow down. Eventually, people reach a stable weight on these GLP-1 RA medications.
For example, in one large study, participants lost an average of 10.6% of their body weight in the first 20 weeks (about five months). Those who continued taking semaglutide lost an additional 7.9% between weeks 20 and 68 (up to about a year and three months). In other words, although they did continue to lose weight, the weight loss slowed down over time.
Meanwhile, sustained weight loss for up to two years has been demonstrated with the continued use of liraglutide.
Some people become concerned when they reach this weight loss plateau. They may believe that the GLP-1 RA medication has stopped working, because they aren’t losing weight anymore. However, this weight loss plateau is normal and expected. If a person were to continue to lose weight indefinitely, they would eventually become unhealthy. It’s actually a good thing for your weight to stabilize at some point.

Once they’ve reached a stable weight, remaining on GLP-1 RAs can help many people to maintain a healthy and stable body weight. This is one of the major benefits of long-term use of GLP-1 RA medications such as semaglutide and liraglutide.
However, there are some individuals who are able to successfully maintain their weight after going off semaglutide. Whether this is possible for you will need to be discussed with your treating physician.

Helps control blood sugar
GLP-1 RAs have been shown to control blood sugar effectively, making them an attractive option for many diabetes patients. For example, in a clinical trial in patients with type 2 diabetes (the Sustain 7 Study), an injectable form of semaglutide was shown to lead to a reduction in HbA1c of up to 1.8% over 40 weeks. GLP-1 RAs such as semaglutide, liraglutide, and dulaglutide can be used on their own, in conjunction with lifestyle changes like exercise and taking a healthy diet. If needed, they can also be combined with other diabetes medications (such as metformin).
Reduction in risk of heart disease and kidney failure.
GLP-1 RAs have been shown to be cardioprotective, meaning that they reduce the risk of heart disease. A study has demonstrated that semaglutide helps prevent cardiovascular risks such as cardiovascular death, non-fatal heart attack, or non-fatal stroke, by 20% in overweight and obese adults.

Overall, GLP-1 RA is considered to be safe for long term use. A study has shown a 20% reduction in risk of heart disease and stroke in obese individuals with use of GLP-1 RA medications (semaglutide, liraglutide, dulaglutide). There does appear to be a slightly increased risk of gallbladder disease with longer-term use, although the risk is extremely small.

In addition to helping maintain a healthy body weight, GLP-1 RA helps to keep blood sugar under control. This effect appears to last for the long term. In one study, people taking semaglutide for three years had significantly lower hemoglobin A1c levels than those taking a placebo.
For people living with type 2 diabetes, keeping blood sugar under control can help to reduce the risk of long-term health effects, such as heart disease and kidney disease. In addition, people taking GLP-1 RAs have been found to have significantly lower blood pressure than those taking a placebo.
Recent studies have also found benefits for those experiencing heart failure. Over one year, people taking GLP-1 RAs experienced significantly greater improvements in symptoms and physical function than those taking a placebo. GLP-1 RAs reduced the risk of adverse cardiovascular events (like strokes, heart attacks, and sudden cardiac death) by 20%.
