NOVI Health’s Magnum Program, as featured in CNA:
“In 2022, our National Population Health Survey showed that only 1 in 3 persons with diabetes actually had good control of their diabetes.
And worst still, in younger populations aged 30 to 39, a vast majority, almost 80% of those with diabetes did not have good control.
What it takes to manage the condition actually needs to be very personalised and holistic. That involves the patient, the person himself, in partnership with their care team and their care providers.” – Dr Sue-Anne Toh

*Results are based on a peer-reviewed study of NOVI’s program. Note that individual outcomes may vary.

Regular monitoring of glucose is key to good diabetes control. Compared to glucometers, CGMs monitor glucose conveniently, continuously, and painlessly.
The data gathered by continuous glucose monitoring sensors can provide valuable insights into your daily lifestyle choices and better enable healthcare professionals, like NOVI, to provide actionable diabetes management advice tailored to your unique needs. Examples include:
Suggesting food swaps that reduce blood sugar spikes
Recommending foods that help prevent blood sugar dips
Adjusting your medication based on your unique glucose patterns

Health coaching will help equip you with the knowledge, tools, skills, and confidence to manage your illness. The time you will need to commit to health coaching for chronic diseases will depend on your provider and the disease. Health coaching may be performed in person or remotely, like we do at NOVI, but could depend on your provider.
Health coaching can provide you with benefits such as:
Self-management support, including the promotion of healthy behaviors and lifestyle changes and teaching illness-specific skills
Disease-based knowledge, including answering any questions you may have about your illness, medications, and other treatments
Aiding you in the navigation of the healthcare system, including assistance with locating and engaging in other medical services
Emotional support, which can help you cope with the emotional impact of your chronic illness

Around-the-clock glucose readings offered by CGMs serve little purpose in diabetes management unless you take action to improve your glucose levels. This is where a reliable training and coaching program comes into play.
At NOVI, we work hand-in-hand with patients to create a holistic treatment plan that best suits their unique situation and needs. We use CGMs throughout the process to better understand how patients' glucose levels change in response to their lifestyle choices: from food intake to physical activity. This, in turn, helps us provide more targeted, actionable advice that'll help improve diabetes control.
-
As a testament to NOVI's effectiveness, our coaching programs have resulted in a 0.9% reduction in the average blood glucose levels and a 5.8% loss in body weight over three months. This is based on our peer-reviewed study. A 1% reduction in HbA1c reduces the risk of diabetes-related deaths by 21%, heart attacks by 14%, and amputation or death from peripheral vascular disease by 43%.

You will not be asked to make big uncomfortable changes to your diet and lifestyle. We want to help you make changes that will stick with you, so it is very important that our advice fits as closely to your preferences as possible and progresses at a pace that is comfortable for you.
-
We will suggest food swaps that you will enjoy, and teach you how to make modifications to the food you enjoy so that they can contribute towards a balanced diet for you. If you are someone who enjoys preparing your meals, we can also provide advice on meal prepping to make your meals healthier.
-
Our dietitians will work with you on your food preferences to find a way of eating that is sustainable for you that is also tailored to meet your nutritional needs. We believe that this is key for an individual to make long-term changes to their diet.
We will look at the foods you are eating regularly, help you understand how they affect your health, and provide you with suggestions to make your meals healthier.
If there is a specific diet that you would like to try (e.g. Keto, vegan, Mediterranean), you can discuss this with your dietitian during the program.
-
An exercise plan is not compulsory. While we do encourage participants to keep active, you will not be made to do anything that you are not comfortable with.
The exercise that is recommended will be based on your preferences. It could be as easy as taking a 15-minute walk after meals or doing a 2km run on 2 days of the week. If you’re more of a gym-goer, we will work with you to design a full workout plan that you can use at the gym. These recommendations will evolve as you get fitter and stronger. We will take any existing medical condition or injuries into account when suggesting exercises so that you can become more active safely.

It's essential to note that the response to our program varies based on factors such as the duration of diagnosis, age, and the severity of the condition. We personalize our approach to best suit each individual's needs and circumstances.
