Meet your health coach
You will meet with your personal health coach who will be with you throughout your journey with us. Your health coach will take the time to listen and understand you and your goals. You can be seen in clinic or online.
Get your diabetes starter kit
You will receive 1 FreeStyle Libre sensor so you can monitor your glucose in real-time, and 1 month supply of Glucerna Triple Care - a complete and balanced nutrition supplement designed specially for people with diabetes.
Personalized health plan
Your health coach will design a health plan specially for you, covering nutrition and fitness, tailored for your health goals and to your unique circumstances and health status.
Remote coaching and support
Track your glucose in real-time with your continuous glucose monitor. Through our health app, you continue to get support and coaching outside of your visit.
