Dr Todd On is an Associate Consultant, Family Physician with 13 years of clinical experience in tertiary hospital and community settings. He has cared for patients across age groups, collaborating with multidisciplinary teams to deliver person-centred care for both acute and chronic conditions. Dr On has led multiple quality improvement projects, including a smoking cessation programme and a collaboration to reduce lower limb amputation in diabetic patients. He is also passionate about teaching, serving as a Faculty Tutor and Mentor at the College of Family Physicians Singapore, where he has received multiple teaching awards.

A Commitment to Person-Centred Care

As a Family Physician, Dr. Todd On is committed to providing holistic and continuous care for individuals and families. He believes in building strong patient-doctor relationships and is constantly looking for ways to improve how care is delivered. As a former Teamlet Leader in the NHG Polyclinics, he has experience leading a team of doctors, nurses, and care coordinator to provide comprehensive support for people with chronic conditions.

With 13 years of clinical experience, Dr. On has a deep understanding of primary care needs.

Broad Scope of Practice:

He is experienced in managing a wide range of acute and chronic medical conditions for paediatric, adult, and geriatric patients, with additional experience in women’s health and mental health.

Extensive Primary Care Background:

He has previously spent over nine years practicing in the National Healthcare Group (NHG) Polyclinics, a foundation supplemented by his work in the medical, surgical, and paediatric departments at the National University Hospital (NUH).

Improving Patient Safety and Quality of Care

Dr. On places a strong emphasis on patient safety and has spearheaded initiatives to enhance care quality.

Enhancing Medication Safety:

He has contributed to committees focused on improving medication safety by developing guides for new prescriptions, addressing the risks of taking multiple medications in the elderly (polypharmacy), and creating protocols for medication recalls.

Driving Health Initiatives:

He has guided quality improvement projects, including a cross-institution collaboration with Tan Tock Seng Hospital to improve outcomes for people living with diabetes and promoting smoking cessation through the national IQuit Campaign.

Recognition for Patient Safety:

In recognition of his contributions to safer work practices, he was awarded the National Healthcare Group (NHG) Excellence in Action Award.

Dedication to Community Health

At the start of the COVID-19 pandemic in Singapore, Dr. On volunteered to serve in the foreign worker dormitories. He helped to set up and lead a medical post, providing essential acute, chronic, and mental health services to migrant workers. For his dedication during the nation’s response to the pandemic, he received The Commendation Medal (COVID-19), conferred by the Prime Minister’s Office.

Educating the Next Generation of Doctors

Dr. On is passionate about medical education and is actively involved in training future Family Physicians. He serves as a tutor for the College of Family Physicians (CFPS) and is a former faculty lead. He has also previously held teaching appointments with the Yong Loo Lin School of Medicine (NUS) and the Lee Kong Chian School of Medicine (LKCMedicine). His commitment to teaching has been recognized with multiple awards, including the NUS Dean’s Award for Teaching Excellence and the CFPS College Teachers’ Award.
